Only if you check the text-message box on the form, we may use your phone number to send texts about your appointment request, scheduling, appointment reminders, and related customer service. We do not use it for marketing or promotional messages, and you can request an appointment without agreeing. Message frequency may vary. Message and data rates may apply. Our website is hosted by Netlify. When you send the form, your request is stored in our customer management system (HighLevel, also known as LeadConnector) so the office you chose can contact you, and an internal email notifies that office.
Please do not send sensitive information
Please do not submit Social Security numbers, tax documents, account numbers, or other sensitive financial information through the website form.
Website logs
Like most websites, this site may generate ordinary server and security logs through our hosting provider.
This temporary website does not intentionally use advertising trackers or analytics tools.
